  • As of February 1, carisoprodol (Soma) will no longer by on SFHP formulary, as it is not recommended for long-term use.  If you have patients on long-term Soma, we recommend that you talk to them now about a weaning plan, or transition to other, less addictive muscle relaxants that are on formulary, such as Baclofen, Cyclobenzaprine, and Methocarbamol.
