Updates from October 2009 P&T Committee for Medi-Cal, Healthy Families and Healthy Kids

 











































*SFHP listened to you, our providers!  Based on feedback we received from you, SFHP Pharmacy and Therapeutics (P&T) Committee reviewed acute migraine agents at the October 2009 P&T Committee meeting. The committee added Sumatriptan (any strength) tablets to the SFHP formulary with a quantity limit of 9 tablets per 45 days.

**SFHP promotes quality of care. Because of the potential for drug dependency with chronic use of Carisoprodol, SFHP P&T Committee removed Carisoprodol from formulary effective February 1, 2010. Please use formulary alternatives—baclofen, cyclobenzaprine 10mg or methocarbamol.

NF = Non-formulary
Code-1 = Pharmacy POS override
CT = Contingent Therapy
QL = Quantity Limit
