Members can receive $25 or $50 gift cards (depending on the program) for getting certain tests, screenings and check-up visits with their doctors. The following incentive programs are offered to members to encourage and reward them for getting timely preventive care:

• Well-Child
• Well-Adolescent
• Childhood Immunizations
• Prenatal
• Diabetes

Providers are reimbursed at $20 for each well adolescent visit form submitted indicating that an adolescent member had the required visit. Providers are now able to download the Well Adolescent Visit Submission Form directly from our website!
