Free Webinar Training Resources on Patient Centered Medical Home

In January, HealthSciences Institute launched an exciting new chronic care improvement community featuring free, noncommercial monthly Continuing Education (CE) webinars. These webinars are designed to help nurses, care managers, health coaches, physicians, pharmacists, and other medical home team members put chronic care and medical home models into practice. The sessions are interactive and highlight practical solutions and steps for better medical home performance and improved patient adherence, self-care and lifestyle management.

  • All events are held on Fridays from 10:30am to 11:30am (CST) 
  • Each event is approved for 1.0 hour of continuing education credit for Chronic Care Professional (CCP) recertification
