Healthy Weight Program
SFHP is currently working to improve our Healthy Weight Program in an effort to reach more SFHP members. The biggest change to the program will be that the “Eat Smart Be Active” Program will be phased out as the main offering of the Healthy Weight Program. Any sites that have already participated in the training for this program will continue receiving support from SFHP as needed. Reimbursement funds will be rolled up into HECP and can be coded as Nutrition/Weight Management group classes. We will no longer provide the full 8 week train-the-trainer sessions to new sites. Any new sites interested in this program can still receive program materials and a program overview training from our Coordinator of Health Education and Cultural Linguistic Services. This will provide interested sites with more flexibility by enabling them to tailor the curriculum to their patient population needs and existing obesity prevention programs. SFHP will roll out the new Healthy Weight Program in Fall 2010. We will keep you updated about future opportunities.
If you have any questions, please contact Megan Petrich, Coordinator of Health Education and Cultural Linguistic Services at mpetrich@sfhp.org or (415)615-4420.
