Last year San Francisco Health Plan was the top Medicaid plan in the nation for immunization HEDIS rates and we won the Gold Award for Quality (third year in a row). We could not have done it without your help. The childhood immunization rates reported by HEDIS come from the Recommended Childhood Schedules approved by the American Academy of Pediatrics, the Advisory Committee on Immunizations Practices of the Centers for Disease Control and Prevention, and the American Academy of Family Physicians.

Thank you for continuing to support our efforts in providing quality care for our members. Below are the childhood immunizations that are reported by HEDIS. PLEASE NOTE: the immunizations that are new reported measures this year are underlined.

Recommended immunizations by age 24 months:

• 2 Hepatitis A

• 2 Flu (yearly)

• 3 Rotavirus (only 2 if Rotarix is administered)

• 4 DTaP

• 3 Polio

• 3 Hepatitis B

• 3 Hib

• 4 Pnuemococcal

• 1 MMR

• 1 Varicella (unless documented chickenpox diagnosis)

SFHP will continue to encourage our members to receive all recommended vaccinations. We offer a $50 gift card for members who receive them before turning two.
