Incentives Help You Give Better Care: SFHP Incentive Programs

Strong provider participation is particularly crucial to the success of 3 of SFHP’s 5 incentive programs. Here are descriptions of these programs, and what providers must do to make them work for SFHP members. 

Improving Prenatal and Postpartum Care (two $25 gift cards)
Currently, pregnant members who have had their prenatal checkup within the required timeframe (42 days for new members and 1st trimester for existing members) are eligible for a $50 gift card.  We are transitioning this program to also emphasize post-partum care within the required timeframe (21 - 56 days).  In the future, women are eligible for two $25 gift cards, one for prenatal care and one for post-partum care.  Please look for program details in September.

SFHP needs providers to let pregnant members know about this incentive because members must call in order to receive a card. To request this card members can call (415) 547-7800 or (800) 288-5555



Improving Diabetes Care ($25 gift card)

Members 18-75 years of age with diabetes (type 1 or type 2) who had each of the following screening tests will receive a $25 gift card:

          • Blood pressure
          • HbA1C
          • LDL-C
          • Medical attention for nephropathy
          • Dilate Eye Exam
          • Foot Exam

                                         Members may bring in this card for providers to complete. The card can be mailed or faxed to SFHP.


Improving Well Adolescent Visits (2 movie tickets or $15 gift card)

Adolescents 12-21 years of age who received a comprehensive well-care visit during the calendar year will receive 2 movie tickets or a $15 gift card. They are also entered into a raffle to win a laptop computer or iPod Nano.

Providers submit a list of SFHP members who received a comprehensive well-care visit, and in return providers receive $20 for each member who received a comprehensive well-care visit and is an eligible SFHP member.
