  Seniors and Persons with Disabilities Expansion
June 1st Go Live Date



The State is beginning the mandatory enrollment of seniors and persons with disabilities (those without
Medicare) into managed Medi-Cal in beginning in June 2011.  Any of your Fee-for-Service (FFS) Medi-Cal patients who do not have Medicare will be required to enroll in managed care in the month following their birth month.

  Vision Service Plan (VSP) Benefit Enhancement for SFHP Medi-Cal Members Effective 4/1/11
SFHP recognizes the importance of vision exams for diabetic members to prevent diabetic retinopathy and other complications. In order to provide exceptional care to Medi-Cal members with diabetes, SFHP has decided to allow annual optometry exams for diabetic members only  effective April 1, 2011.

  Care Coordination and Care Management Program
CareSupport is the new SFHP care coordination and care management program that will launch on June 1st, 2011. We recognize that many providers may not have sufficient staff resources to devote to supporting those patients with multiple chronic illnesses who require more time-intensive interventions such as individualized health coaching, treatment adherence teaching and/or linkage with appropriate benefits and key community resources. CareSupport is a short-term care management program to address this gap.

Using qualified interpreters when providing care to limited English-speaking or hearing-impaired patients ensures that providers can communicate important medical information to their patients. It also allows patients to ask questions and participate in conversations about their health.

This year, California’s Managed Risk Medical Insurance Board (MRMIB) added Immunizations for Adolescents as a required HEDIS (Healthcare Effectiveness Data and Information Set) measure for Healthy Families members.

  Help rid the City of hepatitis B by joining the SF Hep B Free Clinician’s Honor Roll
Ten percent of Asian & Pacific Islanders (API) in the United States have chronic hepatitis B, which can lead to liver cancer if left untreated. With its sizable API population, nowhere is the HBV epidemic more pronounced and visible than in the Bay Area, which has the highest rate of liver cancer in the nation.

  2011 Practice Improvement Program
San Francisco Health Plan (SFHP) is excited to announce that there are 25 provider groups and clinics participating in SFHP’s Practice Improvement Program (PIP), which launched January 1, 2011. Click here for a list of participants. PIP was designed to help our network prepare for the upcoming opportunities and challenges involved in implementing health care reform and enrolling Seniors and Persons with Disabilities into managed care.

  Communicating with patients about Weight Management
Addressing the obesity epidemic is a top priority for SFHP. The HEDIS measures for Weight Assessment and Counseling for Nutrition and Physical Activity fit nicely into our efforts to prevent and reduce obesity here in San Francisco. Please use BMI wheels or other tools to help calculate the BMI percentile of your patients.
