Disease Management Coordinator
Major Responsibilities:
- Participates in screening of initial data to identify participants in need of direct disease management.
- Makes initial and follow-up calls to participants and providers to develop the appropriate care plans and coordinates care plans with Disease Management Coordinators , providers and community resources, in accordance with disease management protocols, to ensure that participant’s needs are being coordinated within the health care system.
- Coordinates activities with the current Quality Improvement/Health Education programs and participates in the development of new programs.
- Logs telephone call notes in disease management database.
- Ensures the privacy and security of PHI (Protected Health Information) as outlined in SFHP's policies and procedures relating to HIPAA compliance.
- Makes recommendations for improvement to leadership.
- Other responsibilities as assigned.
Required Qualifications:
- Current unrestricted California RN or LVN license, required.
- Bilingual written and fluent verbal skills in Spanish or Cantonese, required.
- Detail-oriented, highly organized, and able to work with multiple team members and departments.
- Ability to communicate effectively (spoken and written) with internal and external parties.
- Experience with chronic health care conditions in a health care delivery setting – 1 year required, 3 years preferred.
Preferred Qualifications:
- Computer competency in using Microsoft Office applications including Outlook, and Excel. Familiarity with databases such as ACCESS, a plus.
- Knowledge of and experience with state and federal insurance programs, a plus.
- Bachelor’s Degree in nursing, preferred.
