San Francisco is a unique and one-of-a-kind place. Covering only a 7 by 7 mile area, this metropolis is broken down further into neighborhoods that reflect their own ethnic and cultural diversity. San Francisco Health Plan (SFHP) members reflect the City’s great ethnic and cultural diversity.
We were created by the City and County of San Francisco with one purpose: to provide high quality medical care to the largest number of low-income San Francisco residents possible, while supporting San Francisco’s public and community-minded doctors, clinics, and hospitals.
To help achieve our purpose, SFHP values working in a partnership with the City’s communities and community partners. Our Community Relations team is dedicated to leveraging SFHP’s mission in the community to drive awareness and education of health coverage options for underserved San Francisco residents through outreach, training, dissemination of best practices, and building capacity among our community partners.
