Make a Request

San Francisco Health Plan has brochures, health education materials, enrollment information, and much more to share with the groups you serve.  All of our materials are translated in multiple languages, and our enrollment team is bi-lingual. 

Our community services are free of charge, and we can make available:

  • On-site Presentations
  • Brochures about health care coverage program enrollment
  • Plan participation at community events
  • And much more!
