Get a New ID Card


San Francisco Health Plan issues each member an ID card. When you get your member ID card, check all information to make sure it is correct. You may have a new ID card mailed to you at no charge if:


1. Any information is incorrect
2. Your card is lost or stolen
3. You change your Primary Care Provider
4. Your address or other information changes


To request a new member ID card, call our Customer Service at 1(800) 288-5555, Monday–Friday, 8:30am to 5:30pm.

In addition to your SFHP ID card, you should also have a Beneficiary Identification card (BIC) or Medi-Cal card from the State of California. You may need to show that card to get certain services. Call 1(415) 863-9892  if you have any questions, or to get a new BIC card.
