  • Change your family’s primary care provider (PCP)
  • Get a new member ID card
  • Report a problem with your PCP or other health care service
  • Get help filling your prescriptions



Call the Healthy Families Program at (800)880-5305 to:

  • Ask about your monthly premium bills
  • Change your address, phone number, or last name
  • Ask questions about your family’s dental plans



Call San Francisco Behavioral Health Services at (415) 255-3737 or (888)246-3333 (toll free) or 1-888-484-7200 (TDD) to:

  • Get mental health counseling
  • Access a substance abuse counselor



Call Vision Service Plan (VSP) at (800)877-7239 to:

  • Get an eye exam or eyeglasses
