How to Keep Your Coverage

As a Healthy Kids member, you'll need to renew your health coverage every year.

There are 3 easy steps to renew your Healthy Kids membership:

1 

Watch for a letter and renewal form. It will come to you in the mail from San Francisco Health Plan.
2 Fill out all the renewal form and return it as soon as possible with the required documents. It's very important that you return it by the due date, which should be listed on the form.
3 If you move, be sure to call us right away at (800)558-5858, or email your new address to our Member Services Team.  

Your Healthy Kids coverage expires one year after you enroll in the program.  If you don't get your renewal packet in mail at least 60-days from when your health insurance will expire, call us right away at (415)546-9820.

If you did get your renewal packet and have questions about how to fill out the forms, contact our Enrollment Team.  Be sure to include your name, phone number, and a explanation of how we can help you.

We will help you fill out the renewal form over the phone, and even make an appointment for you to come to our office to fill out the form.
