Changing Your Doctor

It's always best to keep the same doctor whenever possible so that he or she gets to know your health history and health care needs.  But sometimes you may need to change your doctor. 

You can request to change your doctor for any reason.  Simply email San Francisco Health Plan Member Services, or call us at (800)288-5555, Monday through Friday, 8:30am to 5:30pm.

Tips on Choosing your New Doctor.  Ask yourself...

  • What neighborhood is best for you?
  • Do you want this doctor to be close to home, school, or work? 
  • Is it easy to get to this doctor by car, MUNI, or BART?
  • Does this doctor and their office staff speak your language?
  • Does this doctor work with a hospital you like?
  • Does this doctor see children of all ages?
