If you have questions about eligibility or if you need help filling out the Healthy Workers Enrollment Form and you are a provider of IHSS, call IHSS Public Authority at (415)243-4477.

If you have questions about eligibility or if you need help filling out the Healthy Workers Enrollment Form and you are a temporary exempt as-needed employee of the City and County of San Francisco, call the Department of Human Resources at (415) 557-4942.

  • Get mental health counseling
  • Access a substance abuse counselor 



Call Vision Service Plan (VSP) at (800)77-7195 to:

  • Get an eye exam or eyeglasses



If you are a provider of IHSS and are eligible for dental services, call Liberty Dental Plan at (888)703-6996 to:

  • Make an appointment with a dentist
