How to Keep Your Coverage

As a Medi-Cal member, how and when you renew your coverage depends on your age and parental status. Please review the steps that apply to your situation.

A       If you are a Medi-Cal member aged 0-21, your Medi-Cal eligibility must be reviewed annually:

Watch for a letter and renewal form. It will come to you in the mail from Medi-Cal.
2 Fill out the renewal form and return it as soon as possible with all the requested documents. It's very important that you return it by the due date, which should be listed in the letter or on the form.
3 If you don't hear back from Medi-Cal within 45 days, call the local Medi-Cal office at (415) 863-9892 and ask about the status of your re-enrollment form.

B       If you are a parent currently receiving Medi-Cal, your eligibility must be reviewed every six months, and you must also re-apply every 12 months:

AT SIX MONTHS, Medi-Cal will send you a letter in the mail asking that you confirm your income. Complete this form verifying your eligibility and send it back by the termination date listed in the letter or on the form.

If you don't hear back from Medi-Cal within 45 days, call the local Medi-Cal office at (415) 863-9892 and ask about the status of your re-enrollment form.

AT TWELVE MONTHS, Medi-Cal will send you a letter and renewal form in the mail.

C       If you are a Medi-Cal member because you are or were pregnant:

Your baby will continue Medi-Cal coverage up to age one, around which time your income will be re-evaluated. 

Your pregnancy is covered up to term, and 60 days postpartum (60 days after your baby is born).  Your income will be re-evaluated around that time for continuation of family Medi-Cal benefits.

Prior to 60-day postpartum you must get your baby's Verification of Birth from the hospital and send it to:
