How do we measure it?

Improving Childhood Immunizations

Measures the percentage of children who had four diphtheria, tetanus and acellular pertussis (DTaP), three polio (IPV), one measles, mumps and rubella (MMR), three H influenza type B (HiB), three hepatitis B, one chicken pox (VZV) and four pneumococcal conjugate vaccines by their second birthday.

Measures the percentage of members who turned 15 months old during the measurement year and who had 6 well-child visits with a PCP during their first 15 months of life.

Measures the percentage of enrolled members 3–6 years of age who received one or more well-child visits during the measurement year

Improving Well Adolescent Visits

Measures the percentage of enrolled members 12-21 years of age who received one or more comprehensive well-care visit during the measurement year

Improving Diabetes Care

 

Measures the percentage of members 18-75 years of age with diabetes (type 1 or type 2) who had each of the following:

  • Hemoglobin A1c (HbA1c) testing
  • HbA1c poor control (>9.0%)
  • LDL-C screening
  • LDL-C control (<100mg/dL)
  • Eye exam (retinal) performed
  • Medical attention for nephropathy

Improving Hypertension and Hyperlipidemia Care

Improving Asthma Care
 

Measures the percentage of members 5 through 56 years of age who have persistent asthma and have been prescribed at least one controller medication.

Improving Women’s Health

 

  • Measures the percentage of women 40 to 69 years of age who had one or more mammograms during the measurement year or the year prior to the measurement year.
  • Measures the percentage of women 21 to 64 years of age who received one or more Pap tests during the measurement year or the two years prior to the measurement year.
  • Measures percentage of women 16 to 25 years of age who were identified as sexually active and who had at least one test for Chlamydia during the measurement year.

Improving Prenatal Care

 

Measures the percentage pregnant members who have had their prenatal checkup within timeframe - 42 days for new members and 1st trimester for existing members
