Do You Qualify?

The first step to enrolling with San Francisco Health Plan is to see if you or your child qualifies for any of our programs. To figure out which program is right, we'll ask a few questions about family income, age, San Francisco residency, and in some cases student or parental status.

Qualification is different for each program, so it's always best to contact us to see which program is right for you. You can do this by emailing our Enrollment Team, or by calling (888) 558-5858, Monday through Friday, 8:30 am to 5:30 pm.

You or your child may qualify for one of our programs if you fit into one of the following categories:

Family Size	Monthly Family Income	Annual Family Income
1	up to $2,600	up to $31,200
2	up to $3,500	up to $42,000
3	up to $4,400	up to $52,800
4	up to $5,300	up to $63,600
5	up to $6,200	up to $74,400
6	up to $7,100	up to $85,200

You may qualify even if you think you make too much money
We have several programs and will help you find the right one
Your children may qualify even if they are not legal residents

If you think you don't fit into one of these categories, or if you are not sure if you do, contact our Enrollment Team. Asking a member of our Enrollment Team is the best way to know for sure if you qualify for one of our health care coverage programs.
