To join San Francisco Health Plan, you must be eligible and enrolled in one of the health coverage programs below, in addition be living in San Francisco. Each program has its own eligibility guidelines. Which program you may be eligible for depends on household income, family size, and age.
Our Enrollment Team provides application assistance for Medi-Cal, CalFresh, Covered California, Healthy Kids HMO, and Healthy San Francisco programs, Staff can also provide assistance for the San Francisco City Option Programs.

If you are not eligible for one of the programs accepted by San Francisco Health Plan, there may be other health coverage options available for you through one of these programs below.
The Service Center is located at 7 Spring Street, between Kearny and Montgomery Streets, where Chinatown and the Financial District meet. Our Service Center hours are Monday through Wednesday and Friday 8:30am to 5:00pm; Thursday 8:30am to 4:00pm. We are also open for walk-in appointments based on availability.
