Care Management
Do you need help talking to your doctor about your health concerns or questions? Do you feel overwhelmed when trying to get care?
San Francisco Health Plan’s Care Management programs are here to partner with you on your journey to health. We work with our members both in person and over the phone to assess your needs and create your care plan. If you feel you or a family member would benefit from this service, please call our intake line at 1(415) 615-4515 and speak with a member of our team. This service is FREE for SFHP members.
In our programs, you can:
- Learn how to manage your medical conditions
- Get referrals for community agencies; including food and behavioral health
- Work with our nurse to get ready for your next appointment
- Work with your care team to better understand your medications
