Each member of Healthy Kids HMO, managed by San Francisco Health Plan (SFHP) must choose a primary care provider (PCP) or a primary care clinic. Our online Provider Search Tool helps you find and choose the right PCP for you or your children. Please pick your PCP or clinic from the Provider Search Tool.

PCPs and clinics are listed by neighborhood. The PCP or clinic you choose will oversee all of your health care needs. If you need specialty care, your PCP or clinic will help you find other health care providers, called specialists, and will refer you to them when needed.

Brown & Toland Physicians (BTP)

Hospital care is provided at:

  • California Pacific Medical Center (CPMC): St. Luke’s Campus

Your health is important to Brown & Toland. To ensure you maintain optimal health, they offer programs that focus on overall health, including care management services and a variety of health and wellness classes. There are more than 250 Brown & Toland Physicians located on hospital campuses and in private offices all over San Francisco. Chinese Community Health Care Association (CHI)

Hospital care is provided at:

  • Chinese Hospital (Obstetrics and Pediatrics at California Pacific Medical Center)

There are over 150 doctors from the Chinese Hospital medical staff. Their doctors speak most dialects as well as many other Asian languages and are located throughout San Francisco. Hospitalization is at the bilingual Chinese Hospital that has 24 hour emergency services. A walk-in Customer Service office is conveniently located at the Hospital Office Building. Community Health Network (CHN) with SF Health Network

Hospital care is provided at:

  • Zuckerberg San Francisco General Hospital and Trauma Center (ZSFG)
  • Laguna Honda Hospital and Rehabilitation Center (LHH)

Community Health Network is made up of independent non-profit community clinics and the San Francisco Health Network (SFHN). Hill Physicians Medical Group (HIL)

Jade Health Care Medical Group (JAD)

The network hospital for Jade Health Care is Chinese Hospital. If your Jade Health Care specialist works with another hospital, such as CPMC or UCSF, you might get needed hospital care at the other hospital. North East Medical Services (NEM)

North East Medical Services (NEMS) is a non profit medical group with seven clinics and individual primary care providers. The staff at NEMS clinics and individual provider sites provides comprehensive, quality health care services with a personal touch. NEMS serves all patients and specializes in a wide range of Asian cultural and ethnic backgrounds. North East Medical Services with San Francisco Health Network (NMS)

UCSF Medical Group (UCSF)

Hospital care is provided at:

  • UCSF Parnassus Medical Center
  • UCSF Mission Bay Medical Center
  • UCSF Mt. Zion Medical Center

No external organization, provider or vendor may use the San Francisco Health Plan (SFHP) logo, name, service mark, trademark, or likeness without obtaining prior written consent from SFHP and agreement to comply with any applicable conditions for use.
