Depression screening tools include, but are not limited to:

Adolescent screening tools (12-17 years):

  3. Patient Health Questionnaire (PHQ-9)
  4. Beck Depression Inventory-Primary Care Version (BDI-PC)
  5. Mood Feeling Questionnaire (MFQ)
  6. Center for Epidemiologic Studies Depression Scale (CES-D)
  7. Pediatric Symptom Checklist (PSC-17)

Adult screening tools (18 years and older):

  1. Patient Health Questionnaire (PHQ9)
  2. Beck Depression Inventory (BDI or BDI-II)
  3. Center for Epidemiologic Studies Depression Scale (CES-D)
  4. Depression Scale (DEPS)
  5. Duke Anxiety-Depression Scale (DADS)
  6. Geriatric Depression Scale (SDS)
  7. Cornell Scale Screening
