Health Homes

The Health Homes program is a new Medi-Cal benefit offered to the top 3-5% most complex, highest utilizing members as defined by the Department of Health Care Services (DHCS). This benefit aims to:

  • Increase coordination between medical and behavioral health services/systems
  • Create infrastructure to support multi-system coordination and care delivery
  • Address homelessness/unstable housing of eligible members

Community Based Care Management Entities (CB-CMEs) will be providing comprehensive care management, care coordination, health promotion, comprehensive transitional care, individual and family support services, and referrals to community social supports. The following provider groups are currently serving as CB-CMEs:

  • Kaiser Permanente
  • Marin City Health & Wellness at Bayview Hunter’s Point
  • North East Medical Services (NEMS)
  • Mission Neighborhood Health Center
  • St. Anthony Medical Clinic
  • CCM San Francisco Health Network
  • Mission Mental Health
  • Citywide Case Management

Members are assigned to the CB-CME connected to their PCP whenever possible. Members who are eligible for the benefit and are not assigned to one of the above provider groups for primary care will be served by San Francisco Health Plan’s Care Management Team as their CB-CME.
