Member Centered Programs

San Francisco Health Plan is dedicated to improving the health outcomes for our members. We offer several programs and services to help members manage, control, and maintain their health. These programs are as follows:

CalAIM Incentive Payment Program – Through the Incentive Payment Program (IPP), SFHP has provided grants to contracted Enhanced Care Management (ECM) and Community Support providers to address gaps in delivery system infrastructure and support the expansion of ECM and Community Support services in San Francisco.

Due to limited funding, SFHP is not currently accepting new IPP applications. Please check back in the future for updates.

Care Management – San Francisco Health Plan offers a variety of programs with a multidisciplinary team that can work with members both in person and over the phone to develop and implement patient centered care management plans.

Doula Services – Doula services will be provided under the federal authority of Preventive Services as a State Plan benefit in both fee-for-service and managed care delivery systems. The Department of Health Care Services (DHCS) will cover doula services for prenatal and postpartum visits, and during labor and delivery, miscarriage, or abortion, starting January 1, 2023. Services could be at the beneficiary’s home, as part of an office visit, in a hospital, or in an alternative birth center. Services include health education, advocacy, and physical, emotional, and nonmedical support.

Member Incentives – San Francisco Health Plan offers member incentive programs to encourage members to have appropriate preventative care and services.

Sparkler – San Francisco Health Plan is partnering with the Department of Early Childhood (DEC) to make the Ages & Stages Questionnaires® (ASQ) more accessible and easier to complete using the Sparkler app. Sparkler is a FREE, evidence-based, mobile family engagement platform, available in Chinese, English, and Spanish to San Francisco providers and families of children aged 0 to 5. The ASQs on Sparkler are the same as the ASQs often given by educators or pediatricians except that they are digitally enabled via smartphones or tablets.
