How to Get a Vaccine Appointment for SFHP Members

All SFHP members 5 years and older are eligible to get the vaccine now.

COVID-19 Vaccine for Children 5-11 Years Old

The CDC now recommends that children between the ages of 5 and 11 years get the Pfizer COVID-19 vaccine.

Getting the vaccine will help keep your child from getting seriously sick if they do get COVID‐19 and help keep them from spreading COVID‐19 to others. This vaccine will help protect your whole family and the community.

  • The vaccine is safe based on clinical trials.
  • Your child may have no side effects. Some people may have some mild side effects, which are normal signs that their body is building protection. The most common side effect from the vaccine is a sore arm. Sometimes a person may have a headache or a fever but they should go away in a few days. Call your child’s Primary Care Provider if severe allergy reactions occur, which is rare.
  • A parent or a legal guardian will need to consent for the vaccine, either when making the appointment or at the time of the vaccination except under certain limited circumstances.

COVID-19 Booster Shots

Booster shots are now available for people who got Pfizer, Moderna, or Johnson & Johnson vaccines. COVID-19 booster shots help to keep immunity strong and increase protection against COVID-19.

When are you eligible for a booster shot?

If you received the Pfizer COVID-19 vaccine, you should get a booster if you:

  • Are 12 years of age and older
  • Got your second dose of the Pfizer vaccine at least 5 months ago

If you received the Moderna COVID-19 vaccine, you should get a booster if you:

  • Are 18 years of age and older
  • Got your second dose of the Moderna vaccine at least 5 months ago

If you received the Johnson & Johnson COVID-19 vaccine, you should get a booster if you:

  • Are 18 years of age and older
  • Got your dose of the Johnson & Johnson vaccine at least 2 months ago

If you have already gotten your COVID-19 booster, you do not need to do anything further. Thank you for getting the vaccine.

Home vaccine is an option if it is hard for you to get to a vaccine site. Ask if interested.

If you have any questions about the vaccine and your health, please talk to your primary care provider (PCP). Your PCP is the doctor, nurse practitioner, or physician’s assistant who is in charge of your health care.

It Pays to Get the Vaccine for COVID-19!

SFHP Medi-Cal members who get fully vaccinated between September 1, 2021 and February 28, 2022 can earn a $50 Visa gift card!

This is for eligible SFHP Medi-Cal members 5 and up.

Members don’t have to send anything to SFHP. SFHP will mail the gift card to you about 5-8 weeks after you get your last vaccine dose. Make sure SFHP has the right address for you!

To get the $50 gift card:
  • You must be eligible to get the COVID-19 vaccine.
  • You must get fully vaccinated between September 1, 2021 and February 28, 2022
  • If you have not received a COVID vaccine, to get fully vaccinated you must:
    • Get both doses of either Pfizer or Moderna vaccine
    • Or

    • Get the one-dose Johnson & Johnson vaccine during that time
  • If you have already received one dose of Pfizer or Moderna vaccine, you must get your second dose between September 1, 2021 and February 28, 2022  Read more »
