Member Incentives

SFHP understands that you may be experiencing difficult times due to the COVID-19 pandemic. If you need support to find free food, shelter, medical aid, & help with neighborhood problems in San Francisco please visit freeprintshop.org for information in English and Spanish.

 

Incentives Available to Members

For eligible Medi-Cal members 18+ who have been diagnosed with diabetes or hypertension.

Eligible members who receive a wellness visit checkup within the required time frame (within the eligible campaign year).

Member Incentive Card

For Medi-Cal ages 0 through 15 months.

Children who receive 5 well-child visits during the first 15 months of life.

Members receive mailed information once each year.

For Medi-Cal pregnant members.

Pregnant members who receive a prenatal checkup within the required time frame (42 days for new members and first trimester for existing members).

Members can call SFHP to request a submission form.

For Medi-Cal members who have given birth.

Members who receive a postpartum health visit between three and eight weeks after delivery.

Members can call SFHP to request a submission form.

 

The Childhood Immunization Incentive Program will end on June 30, 2020. If you received this form in the mail, please return it to SFHP by mail or you may have your provider fax it to us. SFHP will process your filled out form for this program as usual.

  • We will process your incentive form within 30 days of submission.
