Member Incentive Reminder

Member Incentive Card

For Medi-Cal and Healthy Kids HMO age two and under.
Members receive mailed information at 13 and 17 months of age.

Children who receive the following shots by age two:

  • Four DTaP
  • Three Polio
  • Four Pneumococcal
  • Three HiB
  • Three Hep B
  • Varicella
  • One Hep A
  • MMR
  • Two Rotavirus

Member incentive card

For Medi-Cal and Healthy Kids HMO ages three through six.

Children who receive a well-child visit during the calendar year.

Members receive mailed information on their birthday month each year.

Member incentive card

For Medi-Cal pregnant members.

Pregnant members who receive a prenatal checkup within the required time frame (42 days for new members and first trimester for existing members).

Members can call SFHP to request a submission form.

For Medi-Cal members who have given birth.

Members who receive a postpartum health visit between three and eight weeks after delivery.

Members can call SFHP to request a submission form.

Member incentive other screenings

Member incentive dilated eye exam

For Medi-Cal, Healthy Workers HMO with diabetes ages 18+.

*One for dilated eye exam, one for all other screenings

Members with a diagnosis of diabetes (type 1 or type 2) who receive each of the following screening tests:

  • Blood Pressure Check
  • HbA1c Test
  • Monitoring for Nephropathy

  • Retinal Eye Exam (once a year or if members had a retinal eye exam in the prior calendar year and had a negative test result for retinopathy)

For Medi-Cal, Healthy Kids HMO, and Healthy Workers HMO with Asthma.

Members with a diagnosis of asthma who complete an Asthma Control Test with their PCP during the calendar year.

Members receive a live outreach call and mailed information once a year.

Member incentive card

For Medi-Cal members with hypertension ages 18+.

Members with a diagnosis of hypertension who receive a blood pressure check with their PCP team and who complete the Healthy Action Plan during the calendar year.

Members receive mailed information once a year.

  • We will process your incentive form within 30 days of submission.
  • Call SFHP Customer Service at 1(800) 288-5555 if you have not gotten a response within 5 weeks of submission.
  • Providers can fax your incentive forms directly back to SFHP at 1(415) 615-4547.
