Each member of San Francisco Health Plan (SFHP) must choose a primary care provider (PCP) or a primary care clinic. Our online Provider Search Tool helps you find and choose the right PCP for you and your family.
Please pick your PCP or clinic from the Provider Search Tool. PCPs and clinics are listed by neighborhood. The PCP or clinic you choose will oversee all of your health care needs.
If you need specialty care, your PCP or clinic will help you find other health care providers, called specialists, and will refer you to them when needed. Choosing a PCP or clinic is simple.
