Access your health information anytime from anywhere you have internet access with our Member Portal. Past health visits, ID Card requests, your medicines, provider information, and more are now easy to find through our safe and secure online portal.

See your past health visits, a map of your provider’s office, and more. You can bring your Personal Health Record with you when you visit your doctor.
Change Your PCP
To change PCP, use the My Health Plan dropdown and select PCP Change Request. PCP changes will be effective on the first day of the following month. Watch Video
Request ID Card
You can see your ID Card, ask for a new one, print a temporary card, or save a picture of your card on your phone. Watch Video
Search for Providers
Our improved provider search tool allows you to find providers in your network or any other SFHP network. Look up providers by name or the type of provider including clinics, specialists, and more. To search for pharmacies, go to Medi-Cal Rx’s homepage.
Health Education Library
Our portal has a library on many wellness topics. There are interactive tools for health and fitness, medications, pregnancy, a symptom checker, and learning centers to help you make better health decisions.
Download Member Materials
Now, you can view and download the most current member materials and newsletters through our member portal or on sfhp.org.
How to Sign Up and Sign In
- Start at sfhp.org and click the Member Portal Login button at the top of the home page.
- You’ll be directed to the SFHP Member Portal Login Page. Click New User Sign Up button.
- You’ll be asked for your Member ID, Last Name, and Birth Date and then to create a password and set up security questions.
- Now you’re ready to get started. Enter your User Name and Password and click Sign In to begin using the SFHP Member Portal.
The SFHP Member Portal is available to our members 12 years of age or older. SFHP members under the age of 12 will need an Authorized Representative to sign up.
