Incentives Available to Members

Eligible members can earn gift cards by going to certain health visits. Visit the Health Rewards page for a list of all current incentive programs and eligibility criteria.

You may request a presentation to your staff about SFHP’s incentive programs. For more information contact the Health Education Program.

The member or the provider do not have to fill out any forms to get the gift card.

The gift card will be automatically mailed to the member after 8 weeks following their health visit(s).
