How to Enroll in SFHP Care Plus
You can enroll in SFHP Care Plus beginning October 15, 2025.
If you get Medicare and later qualify for Medi-Cal—or if you have Medi-Cal and turn 65—you can sign up once you have both.
Starting in 2026, people with both Medicare and Medi-Cal (dual eligibles) can join or switch D-SNP plans every month.
How to Apply
Contact us today if you live in San Francisco County and:
- You have Medicare and Medi-Cal coverage
- You will be turning 65 and qualify for Medicare and Medi-Cal

You can visit the SFHP Service Center for in-person help and information on health care options for you and your family. Our Enrollment Team is available, by appointment, to help with your Medi-Cal, CalFresh, Covered California, SF City Option, Healthy San Francisco, and many other programs.
