Looking for a new primary care provider (PCP) or trying to find a specialist for a specific health problem?

Whether you are an SFHP Medi-Cal member, SFHP Care Plus (HMO D-SNP) member, or a Healthy Workers HMO member, you can use the Provider Search Tool to choose the right PCP or other provider for your care.

How to Use the Provider Search Tool

SFHP partners with 4,000+ providers to offer you many choices for your care. The Provider Search Tool can help you find what provider may be the best fit for you.

You can sort providers by:

1. Your Member ID, to find in-network providers*
2. Your medical group or care network, to find in-network providers
3. Zip code and distance, to find providers near you
4. Plan type, to find in-network providers
5. Provider type (PCP, specialist, behavioral health, etc.)

Choose how you want to sort. Then click “Search Providers” to find a list of covered providers.

*You can check your Member ID card to see what medical group you have. Your medical group tells you what providers are in your network. Learn more about how to use your Member ID.

Choose the Right Primary Care Provider for You

Your primary care provider (PCP) is your personal doctor if you need a check-up, want advice about a health problem, or get sick or hurt. Your PCP may be one person (like a doctor, OB/GYN, or physician’s assistant) or a clinic. They offer preventive care to help you stay healthy and feel your best.

All members must choose a PCP, or you will be assigned one. When choosing or changing a PCP, ask yourself:

• Is my PCP close to where I live or work?
• Is it easy to visit my PCP by car, MUNI, or BART?
• Does my PCP speak my language?
• What does my PCP specialize in treating?
• Does my PCP work with the hospital I prefer?

Search for Specialists, Clinics, and More

Need expert care for a health problem like asthma, diabetes, or heart disease? Your PCP can refer you to a specialist. Specialists are doctors who work in one area of health care. Talk to your PCP to learn if you may need specialty care.
