SFHP Care Plus partners with North East Medical Services (NEMS) and All American Medical Group (AAMG) (effective 10/1) to offer excellent Medicare and Medi-Cal coverage. SFHP Care Plus connects you to a trusted network of doctors and other health care providers throughout San Francisco. You can get primary care, specialty care, and other health services close to home and in your preferred language.

Getting Needed Care with Your Primary Care Provider (PCP)
Your primary care provider (PCP) is your personal doctor if you need a check-up, want advice about a health problem, or need treatment when you are sick or hurt. Your PCP is part of the NEMS care network, and they will help coordinate your care and help you get any hospital services you may need.
Check your Member ID Card to find the name, phone number, and address of your PCP.

Schedule Your Annual Wellness Visit (AWV)
Every year, please make sure to schedule your Annual Wellness Visit (AWV) with your PCP. We cover this visit at no cost to you.
The AWV is an important way to get to know your PCP. During this visit, you can:
- Share more about your health needs and goals
- Create a prevention plan together based on your health risk factors
- Find support for food, housing, transportation, and more
