How Your Care Manager Supports Your Health
Your Care Manager is your go-to resource for getting connected to care and support. They coordinate your health and social needs with all your providers, and offer ongoing, personalized support.

- Access services like In-Home Supportive Services (IHSS), medically tailored meals, and behavioral health resources

When it comes to health care, we know how important it is to have someone you trust by your side. Your Care Manager will be with you throughout your health journey to help you navigate providers, prescriptions, and make it easy to get needed care.
Building a Care Plan with Your Care Manager
Complete Your Health Risk Assessment (HRA)
Each year, you must complete a Health Risk Assessment (HRA). Your Care Manager can help you complete this assessment. The HRA helps your care team learn about your health and social needs, so that together, you can create a personal care plan.
- Housing needs
- Transportation needs
- Medication needs
- Behavioral health needs
After you complete the HRA, you’ll meet with your Care Manager and care team. Your care team may include you, your PCP, other providers, caregivers, and family members.
Your Individualized Care Plan (ICP)
Together with your Care Manager and care team, you’ll use your HRA to make an Individualized Care Plan (ICP) based on your health needs and goals. The ICP is a written plan that details what services you will get to stay healthy, what your health goals are, and the timeline for getting the care you need.
The ICP isn’t just a once-a-year plan – it’s the ongoing basis for your care. Your Care Manager will have regular meetings with you, your providers, and your family to talk about how we are achieving your goals and helping you get needed care.
