SFHP is the only local and public health plan in San Francisco. We know our city and the people who live here. For more than 30 years, we’ve delivered low-cost, high-quality health care for our community.
Now, we will be offering a new way to get the care you need, made easy: SFHP Care Plus.
SFHP Care Plus is our brand-new plan, made for people who have both Medicare and Medi-Cal. This is known as a Dual Eligible Special Needs Plan (D-SNP). With SFHP Care Plus, all your Medicare and Medi-Cal benefits are conveniently in one plan, along with extra support to help you get the care you need.
What is a D-SNP?
A Dual Special Needs Plan (D-SNP) combines your Medicare and Medi-Cal coverage, making it easy to get coordinated care that is tailored to your health needs. With a D-SNP, one plan covers all your benefits, and all your providers work together to give you the best care.
Who is Eligible?
To sign up for a D-SNP plan, you must qualify for both Medicare Parts A and B, and currently be enrolled in Medi-Cal.
To qualify for Medicare, you must be:
- 65 and older, or
- Under 65 with a qualifying disability
You must also be a US citizen, or have legally lived in the US for at least 5 years.
Current SFHP members are enrolled in Medi-Cal. If you are not an SFHP member right now, find out if you qualify for Medi-Cal by going to sfhp.org/qualify.

Enrollment for SFHP Care Plus Starts Soon!
The 2025 Medicare Annual Enrollment Period runs from October 15th to December 7th, 2025, so stay tuned – we’ll share more information about SFHP Care Plus benefits and enrollment in early October.
