Your PCP is your main doctor or health care provider who helps you stay healthy and handles your care. Unless it’s a health emergency, your PCP is the first person you should call when you need a yearly check-up, want help with a health problem, or if you get sick or hurt.


To contact your PCP, check your Member ID Card to find their name, address and phone number.

How Your PCP Can Keep You Healthy

Your PCP helps arrange and provide your preventive, non-emergency health care. They are your go-to person for all your health needs and questions.

Your PCP can help you:

  • Treat common illnesses
  • Find health problems early
  • Manage your health conditions
  • Get vaccines (shots) to prevent infections like the flu
  • Find support with food, housing, transportation, and more
  • Get a referral for specialty care, if needed

Schedule a Yearly Check-Up with Your PCP

It’s important to see your PCP for a yearly well-check visit, even if you are healthy. It’s most important for kids, pregnant people, and people with chronic (long-term) health problems like asthma or diabetes. When you first join SFHP, you should visit your PCP for a check-up within the first 120 days (4 months). Seeing your PCP now can help you get to know them and get care more quickly in the future.

Getting a wellness check with your PCP can help you to:

  • Live longer
  • Lower your risk for health problems
  • Stay fit and active
  • Have a better quality of life

 
During a wellness check:

  • Your vital signs will be checked (such as weight and blood pressure).
  • Your PCP will ask about you and your family’s health.
  • You can ask any questions or share concerns about your physical or mental health.
  • Your PCP can connect you to a health class or help with food, housing, transportation, and other support.

Get Help in Your Language

Some cultures may have many ways of taking care of health needs. Some people use natural remedies while others visit a doctor often. It is okay to feel nervous if going to a doctor is new for you. Your PCP is there to help you!

You can find a PCP who speaks your language by using the SFHP Provider Search Tool.

When you schedule your appointment, you can also ask for an interpreter. The interpreter will be with you in person or on the phone during your PCP appointment. Remember, it’s your right to get health care in the language you need. Learn more about getting help in your language with SFHP.

Changing Your PCP

If your PCP is not a good fit for you, you can choose a new provider.

Before you pick a new PCP, ask yourself:

  • Is this PCP close to where I live or work?
  • Is it easy to get to this PCP’s office by car, MUNI, or BART?
  • Does this PCP speak my language?
  • What does this PCP specialize in treating?
  • Does this PCP work with my preferred hospital?

Need a Ride?

With SFHP, you can get transportation to and from covered health visits, like visiting your PCP or picking up a prescription.

SFHP partners with Modivcare to offer both Non-Emergency Medical Transportation (NEMT) and Non-Medical Transportation (NMT). You can request a ride by using the Modivcare App on your phone or by calling Modivcare at 1(855) 251-7098.

Get Online Care 24/7 with Teladoc

Your PCP is your first point of contact for any health needs you have. If your PCP isn’t available, you can get online care 24/7 for simple, non-emergency health problems through Teladoc.

Teladoc connects you with a provider online or over the phone. They can help you with issues like:

  • Cold or flu symptoms
  • Joint aches or pain
  • Skin problems

*If you win the raffle, you will automatically be sent a $50 gift card in the mail. Raffle winners will get the gift card 5 months after they first become an SFHP member. You will not be told if you do not win. Your first health check-up must be within 120 days (4 months) of joining SFHP to be eligible for the gift card raffle. You do not have to fill out any forms to get the gift card. Gift cards cannot be used to buy alcohol, tobacco, or firearms.
