Get help from someone with the same culture, language, or health experience

Medi-Cal has a new health benefit called Community Health Workers (CHW). CHWs are your partners to make sure you get the health care you need. They are leaders in the community who support you.

A CHW has different job titles, such as:

  • Promotores
  • Community health representatives
  • Navigators
  • Violence prevention professionals

CHWs can help you learn more about your health and make health care easier for you. They often share the same language, health condition, or have been through the same experiences as you. CHWs can be a great support to make healthy lifestyle changes.
They can meet with you online or in person. A CHW can help you:

  • Make appointments
  • Talk with your primary care provider* (PCP)
  • Get health education
  • Connect you to benefits (like translators, transportation, food)
  • Create a plan to meet your health needs
  • Navigate housing

You can get a CHW if you have a certain health problem, such as:

  • Need help getting routine care, like vaccines
  • Have a chronic condition, like diabetes or asthma
  • Want to quit smoking, vaping, or chewing tobacco
  • Have trouble with drugs or alcohol
  • Are dealing with violence

You can get a CHW at no cost if you are a Medi-Cal member. Ask your PCP if you can get a CHW to support you or a family member. Learn more at Community Health Worker Services.
