Kaiser Foundation Health (Kaiser) will have a direct contract with DHCS effective January 1, 2024.
Kaiser will now be your Medi-Cal health plan starting on January 1, 2024. Your Medi-Cal health plan will no longer be San Francisco Health Plan (SFHP). You do not need to do anything to stay with Kaiser Permanente. You will be automatically enrolled in Kaiser Foundation Health Plan for your Medi-Cal coverage.
Do you have SFHP Medi-Cal with Kaiser as your Provider Network?
On January 1, 2024, you will be enrolled directly with Kaiser Permanente. There will be no changes to your medical coverage or benefits. You will no longer be a SFHP member. Kaiser will manage your Medi-Cal benefit. Your primary care provider* (PCP) and hospital at Kaiser will remain the same.
*Your primary care provider (PCP) is your main doctor, physician assistant, or nurse practitioner.
Members currently assigned to Kaiser’s Provider Network through SFHP
You are not required to take any action. DHCS and SFHP will transition all current Kaiser-SFHP members to Kaiser Permanente’s direct Medi-Cal line on January 1, 2024. You should have received 30 and 60-day notices from Kaiser in November and December 2023 informing that you will be automatically enrolled into Kaiser’s Medi-Cal Plan, starting 2024.
Does this change affect my Medi-Cal eligibility?
No, this does not change anything about how you qualify for the Medi-Cal program. This also does not change your Fee-for-Service (FFS) Medi-Cal benefits or your Medi-Cal Rx benefits. There is no need to call your eligibility worker unless you need to update your personal information with the county.
Will my Medi-Cal benefits change?
No, your Medi-Cal benefits will not change. There will be no change in the services you have from the Medi-Cal network providers. You can continue to get your Medi-Cal services from the same Kaiser Permanente providers.
Members with both Medi-Cal and Medicare (dual eligible) and are enrolled in Kaiser Medicare Advantage
You do not need to take any action. Medi-Cal will automatically enroll you into the Kaiser Medi-Cal health plan on January 1, 2024. You will be mailed a letter from Medi-Cal in late 2023 about your automatic enrollment in Kaiser.
Continuity of Care
If you are receiving certain services from SFHP, Kaiser will work with you, your providers, and SFHP to make sure you continue to get the care you need. If you are getting services from a provider who is not part of the Kaiser Permanente Medi-Cal provider network, you may be able to continue to see that provider for up to 12 months under Continuity of Care (CoC).
How will SFHP and Kaiser determine which members remain enrolled beginning January 1, 2024?
SFHP will work with Kaiser to identify Kaiser Medi-Cal assigned members as of November month-end enrollment. A list of members will be provided to DHCS by December 4, 2023. Members on the list will be transitioned to Kaiser effective January 1, 2024 unless they actively choose to remain with existing SFHP managed care plan.
How can I stay with SFHP?
If you are currently with SFHP-Kaiser and would like to keep SFHP as your Medi-Cal Managed Care plan, please reach out to SFHP Customer Service at 1(415) 547-7800 to confirm your decision. SFHP will enroll you into another SFHP Medical Group. Please note, you will not have access to a Kaiser PCP or any other Kaiser affiliated physician/hospital. You will be assigned to a new PCP with SFHP.

For new Medi-Cal members beginning in late 2023:
- Medi-Cal Health Care Options will send all new members joining Medi-Cal an MCP Choice Packet at the time of initial eligibility; starting December 2023, members may actively choose between Anthem, Kaiser, and SFHP, with active choice of Kaiser subject to eligibility criteria and Medi-Cal Matching Plan policy.
- Members that do not make an active choice will be automatically enrolled into an MCP based on default assignment.

By December 1: You should have received a letter from Medi-Cal Health Care Options telling you more about Medi-Cal changes and options.
First week of January 2024: You will get a new member packet from Kaiser Medi-Cal plan.
