Enhanced Care Management & Community Supports

Enhanced Care Management (ECM) gives you extra help to manage your health care and social needs. With ECM, you will get an ECM provider and lead care manager.  

They can help you to:

  • Make appointments
  • Answer questions about your medicine
  • Get transportation
  • Get support with healthy food and help to find housing

Community Supports (CS) are special care options you can use to prevent costly visits to the hospital. SFHP offers CS for: 

  • Housing Transition Services. To help you find a safe house to live in. You can also get help with moving or help to keep your current home.
  • Medical Respite. This is where you can heal from an injury or illness, a short-term bed, food, and mental health care.
  • Medically Tailored Meals. These are meals you can get at no cost. They are made to keep you healthy.
  • Sobering Centers. This is a safe place to get sober with a short-term bed, food, shower, and other social.

Talk with your primary care provider* (PCP) to see if you can get ECM or CS.
*Your primary care provider (PCP) is the doctor, physician assistant, or nurse practitioner in charge of your health care.

  • Customer Service: If you need help or have questions, call SFHP Customer Service at 1(415) 547-78001(800) 288-5555 (toll-free), or 711 (TTY), Monday–Friday, 8:30am–5:30pm.
  • Interpreter Services: You can get an interpreter in-person or by phone for your health visits. When you make an appointment, ask for an interpreter at the same time.
  • Need a Ride? SFHP can help you get transportation to any medical appointment covered by Medi-Cal. Ask your provider or call Customer Service.
