Taking care of your health is one of the best gifts you can give yourself. If you are an SFHP Care Plus (HMO DSNP) member, you can get an Annual Wellness Visit each year at no cost to you. It’s a simple and supportive way to stay healthy and catch problems early.

What Is an Annual Wellness Visit?

An Annual Wellness Visit is a special check-in with your primary care provider (PCP). Your PCP is your personal doctor if you need a check-up, want advice about a health problem, or get sick or hurt.
An Annual Wellness Visit is not the same as a check-up when you are sick. Instead, it focuses on your overall health, your daily life, and your future health goals.
This visit helps you and your doctor make a plan to stay healthy, safe, and strong.

Why This Visit Matters

Even if you feel fine, this visit is still important. Your health can change slowly over time. The Annual Wellness Visit helps you and your doctor stay ahead of problems and keep you feeling your best.

At your visit, you can:

  • Check how your body and mind are doing
    Your doctor may ask questions, do simple tests, and see how you are feeling emotionally and physically.
  • Review your medicines
    You and your doctor will look at what you take and make sure everything is working well for you.
  • Talk about health screenings
    Your doctor may suggest tests that help catch problems early.
  • Learn ways to stay active and strong
    You can talk about exercise, food, sleep, and more.
  • Get support for long-term health conditions
    If you have conditions like diabetes, asthma, or high blood pressure, this visit can help you manage them better.
