ID Cards
San Francisco Health Plan issues each Member an ID Card. When you get your Member ID Card, check all information to make sure it is correct. You may have a new ID Card mailed to you at no charge if:
- Any information is incorrect
- Your card is lost of stolen
- You change your primary care provider
- Your address or other information changes
