Stop Diabetes Before It Starts!

With the Diabetes Prevention Program from SFHP and the YMCA

Our Diabetes Prevention Program is a lifestyle modification program designed for adults aged 18 or older who are overweight and at high risk for type 2 diabetes. The program supports participants to reach a healthier weight and achieve an active lifestyle to reduce diabetes risk.


As a participant in the our Diabetes Prevention Program your patient will be led by a trained Lifestyle Coach and spend one year working together with a small group of people with common goals.

In each session they’ll learn strategies for healthy eating, increasing physical activity, and losing a modest amount of weight to reduce diabetes risk.

If they’re at risk for type 2 diabetes, they can make small, measurable changes to reduce their risk and help them live a happier, healthier life.

How It Works

  • Patient attends 26 one hour sessions over 12 months
  • Small group, supportive environment (8‑15 people)
  • Led by a trained Lifestyle Coach on a CDC‑approved curriculum
  • YMCA membership not required
