Transportation Services

San Francisco Health Plan (SFHP) covers rides for members who need transportation to and from Medi-Cal covered services.

Non-Emergency Medical Transportation (NEMT)

SFHP’s NEMT benefit is available to members who are unable to use public transportation due to functional limitations. NEMT assists members who need help getting in and out of the vehicle and/or medical supervision during the trip.

NEMT includes the following modes of transport:

  • Wheelchair Van
  • Gurney/Litter Van
  • Ambulance
  • Air Ambulance

This benefit requires a prescription. You can find SFHP’s Authorization Request for Non-Emergency Transportation (NEMT) and Physician Certification Statement (PCS) form here. The form must be completed by a physician, dentist, podiatrist, mental health provider, substance use disorder provider, or a physician extender (i.e., physician assistant, nurse practitioner, certified midwives, physical/occupational therapist).

Please ensure all mandatory fields on the form are complete.

NEMT Coordination Service

If you are a provider who is not in contact with any of our NEMT providers and would like referral assistance, please submit the completed PCS form to our transportation coordinator. Our coordinator will connect the members with a contracted NEMT provider.

Non-Medical Transportation (NMT)

NMT is private or public transportation to and from covered Medi-Cal services. NMT services are available when a member attests that they have no other way to get to their Medi-Cal covered service.

NMT can be arranged for members who do not require assistance from the driver. No prescription is required to access NMT services.
NMT includes the following:

  • Taxi Vouchers
  • Clipper Card
  • Gas mileage reimbursement (Internal Revenue Service standard mileage rate)

To request NMT please call SFHP’s Customer Service Department at 1(415) 547-7800. Request at least 10 days prior to appointment or as soon as possible for an urgent appointment.

*NMT is a covered benefit for Medi-Cal members only.

Transportation Guidelines

Transportation Available To/From: Medi-Cal covered services, which includes:

  • Medical appointments, including family planning, mental health, and substance use disorder services.
  • Dental appointments.
  • Picking up prescriptions.
  • Picking up medical supplies and equipment.

Drop-Off Timing: Drivers are required to drop members off at least 15 minutes prior to their scheduled appointment. For example, if the appointment is at 10:00am, the member should be dropped off no later than 9:45am.

Companion or Personal Care Attendant: If needed, one other person may accompany the member. When calling SFHP to schedule transportation, let us know that someone will be accompanying them.

Unaccompanied Minors: Transportation is available to unaccompanied minors when California or federal law does not require parental consent for a minor’s service (“minor consent”). For services other than “minor consent” services, SFHP requires written parental or guardian consent for unaccompanied minors to travel alone.
