Care Coordination Incentive Program

All licensed primary care, behavioral health, and vision health providers are eligible to participate in a short-term intervention and incentive program and earn up to two (2) $50 Door Dash cards. To learn more and sign up for the program, click the link applicable to your practice specialty below.

To qualify, you must:

• Complete Attestation 1 by 9/30/2024
• Complete Attestation 2 by 11/15/2024. This will be emailed to participating providers.
