Local Partnerships

SFHP’s mission is to keep the people of San Francisco healthy. We work together with many partners to do this.

We work with the City and County of San Francisco to give members the best support. We also work with other partners, such as:

  • County Behavioral Health Services
  • In-Home Support Services
  • Golden Gate Regional Center
  • County Social Services and Child Welfare
  • Women, Infant, and Children (WIC)
2024 San Francisco Community Health Assessment

SFHP worked with the San Francisco Department of Public Health to create the 2024 San Francisco Community Health Assessment (CHA). This report uses data to highlight health needs and challenges in our city.

SFHP supported this project through:

  • Funding
  • Leadership
  • Data sharing

As part of this process, SFHP’s Member Advisory Committee (MAC) was invited to complete a health assessment survey for the CHA.

Looking Forward
What we learn from this report will help shape the Community Health Improvement Plan (expected December 2025). These findings will help SFHP provide you with more quality care.

SFHP’s Chief Health Equity Officer presented a summary of the CHA results to SFHP’s MAC at the September 3, 2025 meeting and gave a timeline on SFHP’s next steps to address the results.

Memorandum of Understanding

SFHP has a legal agreement, called a Memorandum of Understanding (MOU), with each partner. This helps us to identify different roles. It also helps us to share information so we can provide better care and referrals.

SFHP meets with our partners often to share resources.
