Extra Benefits for Members with Chronic Conditions
SFHP Care Plus offers extra benefits for members with chronic conditions to help you feel your best and meet your health and social goals. If you qualify, you can get support with food, housing, utilities, and transportation. These benefits are known as Special Supplemental Benefits for the Chronically Ill (SSBCI).
Who Qualifies for SSBCI?
Because SSBCI is a special program for chronically ill members, not all SFHP Care Plus members are eligible.
To qualify for these extra benefits, you must:
- Have one or more complex chronic conditions that greatly impact your health
- Be at high risk for hospitalization or other bad health outcomes
- Require extra care coordination
Enrollees must have one of the following chronic conditions to be eligible
- Autoimmune disorders
- Cancer
- Cardiovascular disorders
- Chronic alcohol use disorder and other substance use disorders (SUDs)
- Chronic and disabling mental health conditions
- Chronic conditions that impair vision, hearing (deafness), taste, touch, and smell
- Chronic gastrointestinal disease
- Chronic heart failure
- Chronic kidney disease (CKD)
- Chronic lung disorders
- Conditions associated with cognitive impairment
- Conditions that require continued therapy services in order for individuals to maintain or retain functioning
- Conditions with functional challenges
- Dementia
- Diabetes mellitus
- HIV/AIDS
- Immunodeficiency and Immunosuppressive disorders
- Neurologic disorders
- Overweight, obesity, and metabolic syndrome
- Post-organ transplantation
- Severe hematologic disorders
- Stroke
If you have one of the conditions listed above, you may qualify for SSBCI. Talk to your primary care provider (PCP) to learn more. They can help connect you to benefits and get the care you need. What Benefits Are Covered?
- $20/month for healthy foods/groceries
- Unused monthly funds do not rollover
- Use your SFHP Care Plus Card at participating local stores
- $20/month for utilities (electric, water, heating, etc.)
- Unused monthly funds do not rollover
- Use your SFHP Care Plus Card when paying for utility bills online or over the phone
- 24 one-way trips to non-health locations (church, recreation center, grocery store, etc.)
- This benefit is in addition to your Medi-Cal transportation benefit
Funds for food and rent/utilities will be loaded onto your SFHP Care Plus Card each month. This is the same card you will use to buy approved over-the-counter products like bandages, aspirin, and cold treatments. Learn more about your SFHP Care Plus Card here.
