One ID Card for Getting Necessary Care & Tests
Your SFHP Care Plus Member ID Card helps you access ALL your Medicare and Medi-Cal benefits. You should always carry your Member ID Card with you and show it whenever you get care or pick up a prescription. Do not let anyone else use your Member ID Card.
Do you have a yellow NEMS ID Card? If you prefer, you can continue using your NEMS ID to get care. Whether you choose to show your NEMS ID or your SFHP Care Plus Member ID Card, both cards will let you access all your benefits and services.
- Any information is wrong
- Your card is lost or stolen
- You change your primary care provider (PCP)
- Your address or other information changes
When you get your new ID Card, always check to make sure your information is correct.

Your Member ID Card has your PCP’s contact information, including their name, phone number, and address. If you want to change your PCP, call SFHP Care Plus Customer Service or visit the Member Portal:

Your Member ID Card has other important phone numbers to help you get the care you need and access your supplemental benefits. Check the back of your ID Card to find contact information for your benefits including vision, dental, behavioral health, and more.
