3 Easy Steps to Use the App
- Download the app from the App Store or Google Play by searching “Modivcare.” The Modivcare App is available in the following languages: English, Spanish, Russian, Mandarin, Cantonese, and Polish.
- Make your account by entering your name, email, and mobile number. Then, verify with the code sent to your phone.
- Set your password and log in to start scheduling and managing rides.

What Transportation is Covered?
SFHP Care Plus covers two types of transportation to health visits:
Non-Emergency Medical Transportation (NEMT)
NEMT uses an ambulance, gurney/litter van, wheelchair van, or air transport.
NEMT is for you if:
- You can’t use public transit to get to covered health services
- You need support to get in and out of the vehicle
- You need medical supervision during the trip
For NEMT, your doctor must prescribe the kind of transportation you need before you request a ride. Your doctor can access the Physician Certification Statement (PCS) form:

Non-Medical Transportation (NMT)
NMT uses a car, taxi, bus, or other vehicles to get you to your health visit.
NMT is for you if:
- You don’t have another way to get to covered health services
- You don’t need medical supervision or support from the driver
For NMT, you do not need a prescription, and your doctor does not need to fill out a form.
Important: To make sure your ride is scheduled on time, please remember NEMT must be requested at least five (5) business days before your appointment. NMT must be requested at least ten (10) business days in advance. You can’t make same-day or last-minute ride requests through the Modivcare App. For urgent requests, please call Modivcare at 1(855) 251-7098.

If you have questions or need help requesting a ride, please call Modivcare at 1(855) 251-7098, Monday–Friday, from 8:30am–5:30pm.
For questions regarding your Medi-Cal benefits and services, please call SFHP Care Plus Customer Service. Call 1(415) 539-2273, 1(833) 530-7327 (toll-free), or 711 (TTY), 8:00am–8:00pm. We are open seven days a week from October–March and closed on Saturdays and Sundays from April–September.

Frequently Asked Questions (FAQ)
SFHP Care Plus provides Non-Emergency Medical Transportation (NEMT) and Non-Medical Transportation (NMT) services for your covered health visits.
NEMT can provide more support, like help getting in and out of a vehicle and medical supervision. NEMT requires a prescription from your doctor called a Physician Certification Statement (PCS) before you can schedule a ride to your health visit.
NMT can help if you have no other way to get to health visits, but do not need medical support. NMT does not require a PCS.

NEMT: Ambulance
If you need special medical support during your ride, this service is for you. Ambulance-level transport includes:
- Transfers between medical facilities when you need constant IV medication, health monitoring, or observation.
- Movement from one acute care hospital to another.
- Transportation if you’ve recently started oxygen therapy, and it’s not safe to travel without supervision. (Note: This does not apply if you use oxygen often and are able to carry it yourself safely).
- Transportation when you have chronic conditions, need oxygen, and need health monitoring during the trip.

NEMT: Wheelchair Van
If you need a wheelchair or help traveling between your home, the vehicle, and your destination, this service is for you.
This type of transportation is also for ambulatory members who need support or supervision entering or leaving a building. This includes people with dementia, or people who may feel weak after treatment like dialysis. If clinically needed, your provider can prescribe the wheelchair van level of service (or a higher level of transport) to make sure you are safe and comfortable during travel.

NEMT: Stretcher/Gurney
You may use stretcher (gurney) transportation when prescribed by your doctor. If you can’t sit upright during transport, this service helps you stay safe and comfortable. Also, if your home or clinic has five or more steps and no ramp, you will be transported lying down on a stretcher. This helps prevent injury and stay safe during transportation.

NMT: Ambulatory Door-to- Door
If you walk with a cane, walker, or need a little help from the driver, this service is for you. The driver will help you safely from your door to the vehicle, and again from the vehicle to your appointment location. Please be ready at your pickup time and let the driver know if you need extra support.

NMT: Ambulatory Curb-to-Curb
If you can walk by yourself and don’t need help from your driver, this service is for you. The driver will pick you up and drop you off at the curb, not at your door. For the best and smoothest ride, please provide a reliable phone number where you can be reached before and during your ride. Also, please be ready at the curb five (5) minutes before your scheduled pickup time.

An SFHP Care Plus member, a family member, care coordinator, caregiver, or medical facility staff member can either use the Modivcare App or call Modivcare at
Modivcare will decide your pickup time based on how long it takes to get to your visit.
Be ready for your ride at least 15 minutes before the scheduled pickup time. Drivers only have to wait 5 minutes past your scheduled pickup time, so please be ready when your ride comes.

If there are five or more steps and no ramp at your home or clinic, you will be transported using a stretcher/gurney to keep you safe and prevent you from getting hurt.
If you arranged and paid for transportation due to a Modivcare transportation failure, you may be able to get money back. Please call SFHP Care Plus Customer Service to report you have paid out-of-pocket and submit evidence of payment within 30 days.
Tips for Smooth Scheduling
When requesting your ride, please keep these tips in mind.
- Drop-Off Timing: Drivers must drop members off at least 15 minutes before their scheduled visit. For example, if the visit is at 10:00am, you should be dropped off no later than 9:45am. Modivcare or SFHP Care Plus will confirm your pickup time based on how long it takes to get to your visit. Please be ready for your pickup time.
- Companion or Personal Care Attendant: If needed, one other person can go with you to your visit. When scheduling your visit, let Modivcare or SFHP Care Plus know that someone will be joining you.
