Get Extra Care with California Integrated Care Management (CICM)

Some SFHP Care Plus members can get a benefit called California Integrated Care Management (CICM). CICM services can help you stay healthy, get connected to community support and resources, and reach your health and social goals.

How Do I Qualify for CICM Services?

As an SFHP Care Plus member, you will fill out a yearly Health Risk Assessment (HRA). The HRA helps your care team learn about your health and social needs and how they change over time. The HRA also helps your care team learn if you can get CICM.

If you qualify, your personal Care Manager will help you get CICM services.

CICM is for SFHP Care Plus members who:

  • Have been to the Emergency Room or the hospital many times
  • Need help with housing
  • Need help with mental health or drug or alcohol use
  • Are adults living in the community who might need long-term care
  • Are adults living in a nursing home who want to live in the community
  • Are adults who are transitioning from incarceration
  • Are pregnant or postpartum AND are a race or ethnicity that is at risk for worse health care outcomes during pregnancy
  • Have been diagnosed with Alzheimer’s or dementia, or who have dementia care needs

CICM services can help you to:

  • Improve your health
  • Manage your health conditions
  • Learn more about your medication
  • Coordinate your care with all your doctors and providers
  • Find food, a ride, housing, and other services
  • Connect to community care that might help you and your support system

California Integrated Care Management (CICM) offers extra care at no cost for members with many health needs who want help to improve their health.

CICM provides many services that can help you stay healthy, get connected to community support, and reach your health and social goals.

CICM services can help you:

  • Get connected to community organizations in San Francisco
  • Get extra resources for food, housing, transportation, and more
  • Get services like In-Home Supportive Services (IHSS), medically tailored meals, and behavioral health care
  • Get referrals for specialist care
  • Get the care & oversight you need while in the hospital
  • Understand your medications, and make sure you’re taking them correctly

Your personal Care Manager will help you get the CICM services you need. They coordinate your health and social needs with all your providers, and offer ongoing, personalized support.

To get CICM, you must meet certain requirements. If any of these apply to you, you may be able to get CICM:

  • If you are an adult and do not have stable housing right now
  • If you are an adult and have difficult health issues and you have needed to go to the hospital or emergency department many times over the last 6 months
  • If you are an adult and have a serious mental health condition or struggle with drug or alcohol use
  • If you are an adult living in the community who might need long-term care
  • If you are an adult living in a nursing home and want to live in the community
  • If you are an adult who has been recently released from jail or incarceration
  • If you are pregnant or postpartum AND you are a race or ethnicity that is at risk for worse health care outcomes during pregnancy
  • If you are adult who has been diagnosed with Alzheimer’s or dementia

If you qualify, you can choose whether you want to get CICM services.

With SFHP Care Plus, you can get CICM at no cost.

If you are getting ECM services with SFHP at the time you become a SFHP Care Plus member, you will qualify for CICM services. We will make sure you can continue getting the same care you need with SFHP Care Plus.
