Getting Started with Your New Plan
We want to make sure you get the care and support you need for your best health. As a new member, there’s three important things you can do to set yourself up for success:
- Fill out your Health Risk Assessment (HRA).
If you haven’t already, you must complete an HRA in your first 90 days as a member and update it each year. The HRA helps your care team understand your health needs and goals. Together with your Care Manager and care team, you’ll use the HRA to make your Individualized Care Plan (ICP).
Your ICP is a care plan made just for you. It says what services you need, your health and social goals, and a timeline for getting needed care. Your care team will use your ICP to give you personalized care and support. - Book your Annual Wellness Visit (AWV).
Your AWV helps you get to know your primary care provider (PCP) and stay healthy. We cover this visit every 12 months at no cost to you. Call your PCP to make an appointment today. You can find your PCP information here or on your Member ID Card. - Get to know your personal Care Manager.
Your Care Manager is your key contact and go-to resource for getting connected to care. They work with you, our plan, and your care team to coordinate and give you the best care. During your time with SFHP Care Plus, your Care Manager will be with you every step of the way—making sure you’re getting the care you need to meet your goals.

Next Steps for Staying Healthy
Filling out your HRA, going to your Annual Wellness Visit, and getting to know your Care Manager are three essential steps to help you stay healthy and get the best care. But they’re just the beginning! Once you’ve completed those steps, get to know the rest of your plan benefits, materials, and more:
- Explore your covered benefits.
With SFHP Care Plus, you pay $0 for medical services and no monthly premiums. You also have extra benefits at no cost – because complete coverage goes above and beyond yearly check-ups. We offer many supplemental benefits like vision, dental, hearing, and extra benefits for members with chronic conditions (SSBCI) to help you achieve your best health. - Access your health information in the Member Portal.
Visit the Member Portal to see your health history, change your PCP, ask for a new ID card, and more. Not signed up? Go to sfhp.org/careplus-portal to make a new account (or log into your existing account). - Find your member materials online.
Visit sfhp.org/careplus-yourmaterials to view member materials like the Summary of Benefits, List of Covered Drugs, Provider and Pharmacy Directory, and more. You can also request a printed copy of any of these materials by calling SFHP Customer Service, and we’ll mail it to you within 3 business days.
